Scaling Family Index Testing via DHIS2 Tracker Use

Background: Family Index Testing (FIT) is essential for identifying at risk individuals and improving HIV case detection. Methodology: DHIS2 Tracker was configured to streamline Family Index Testing program by enabling patient-level data collection and real-time monitoring. A custom tracker program that registers women and girls aged (10–60 years) in HIV care, listing and linking their children under them to capture their HIV status and other key variables was developed. The FIT program has two program stages; the first stage collected baseline data on the index client while the second stage collected data on each child. Existing data on family size, age of children and HIV status for each child of the index case were collected and analyzed to identify gaps in testing coverage. Multiple virtual trainings spanning the space of one month were deployed to train over 2,000 individuals from different stakeholders to support the onsite step down trainings to HCWs. Results: The intervention significantly improved HIV case detection and linkage to care among children. It streamlined data collection, enabling real-time monitoring and identification of testing gaps. The use of virtual training and continuous mentoring enhanced staff capacity, driving effective implementation. By integrating real-time support, the intervention facilitated collaboration and problem-solving, improving workflow efficiency. It strengthened data-driven decision-making, ensuring targeted interventions for untested children and maximizing resource allocation. Conclusion: Overall, the intervention enhanced programmatic outcomes, supported health system effectiveness, and empowered personnel to deliver better HIV care across Nigeria. Insights from the data informed targeted interventions to improve case finding and linkage to care. This approach demonstrates the value of DHIS2 Tracker in optimizing FIT implementation.
